Your signal strengths are actually pretty good. You may be hitting the jitter/latency limits of EVDO RevA. Amplifier "maybe" — if RSS varies drastically, a 13dBi Yagi like the Wilson Yagi might help. Otherwise you're limited by network.

Clarification: fading doesn't have to take you below receiver sensitivity — just decrease SNR forcing lower modulation. Amplifier improves SNR. If VZW uses 800MHz in your area, leaves won't affect much.

Thanks Thuor. LTE? Spending $200+ on a maybe is too much financial hardship. Callcentric has great support. Trying to convince my wife the setup works.
LTE is "Long Term Evolution" that Verizon has committed to deploy. My parents in India use Vonage on worse connections and optimizing VoIP settings brought quality to discernible levels.
